Mitz: Uw toestemming tot informatie delen met andere zorgverleners

Onze praktijk gebruikt nu Mitz voor het online vastleggen van toestemmingen

Vanaf 26 november 2025 maakt onze praktijk gebruik van Mitz. In Mitz kiest u zelf welke zorgaanbieders u toestemming geeft om uw medische gegevens beschikbaar te stellen. Uw andere zorgaanbieders kunnen deze gegevens dan opvragen als zij deze nodig hebben voor uw behandeling.

Waarom is toestemming geven belangrijk?

Zorgaanbieders zoals de apotheek of huisartsenpost, hebben soms uw medische gegevens nodig van huisartsenpraktijk Reguliersgracht. Zij kunnen u dan beter helpen. Wij mogen uw gegevens alleen elektronisch uitwisselen met een andere zorgaanbieder als u dat goedvindt. Dit heet toestemming.

Vanaf 26 november 2025 maken wij voor deze toestemming gebruik van Mitz. Heeft u ons al toestemming gegeven via de balie, telefonisch, bij uw inschrijving of via Volgjezorg.nl? Dan zetten wij deze toestemming in Mitz. U kunt deze toestemming zien als u inlogt in MijnMitz.nl.

Hoe werkt Mitz?

Ga naar MijnMitz.nl Op die website staat uitgelegd hoe Mitz werkt. U kunt inloggen met DigiD en direct uw keuzes maken voor al uw zorgaanbieders. Deze worden veilig opgeslagen.

Alleen uw keuzes staan in Mitz. Uw medische gegevens niet. U kunt uw keuzes altijd aanpassen. Ze zijn direct geldig.

Our practice now uses Mitz for recording consent online

Starting 26 November 2025, our practice will use Mitz. In Mitz, you decide which healthcare providers you give permission to make your medical data available. Your other healthcare providers can then request this information if they need it for your treatment.

Why is giving consent important?

Healthcare providers such as the pharmacy or the out-of-hours GP service sometimes need your medical information from Huisartsenpraktijk Reguliersgracht. This allows them to help you better. We are only allowed to exchange your data electronically with another healthcare provider if you agree to it. This is called consent.

From 26 November 2025, we will use Mitz to manage this consent. Have you already given us permission at the reception desk, by phone, during your registration, or through Volgjezorg.nl? Then we will transfer this consent into Mitz. You can view this consent when you log in to www.mijnmitz.nl.

How does Mitz work?

Go to www.mijnmitz.nl. The website explains how Mitz works. You can log in with DigiD and immediately make your choices for all your healthcare providers. These choices are stored securely.

Only your choices are stored in Mitz — your medical data is not. You can adjust your choices at any time. They take effect immediately.
